Technically, this would be a quinticycle. A UNIcycle has one wheel, a BIcycle two, a TRIcycle three, and a QUADRIcycle four. Since this has four under the shopping cart portion and one from the bicycle portion, it has five wheels in total and is therefore a quinticycle.

At first I, too, thought he must have had his front wheel stolen in the grocery store parking lot, and found an innovative way to still bike home. Then I saw the video of how he made this. I still often use a bike trailer to pick up groceries. I can’t imagine this solution working better than having the original front wheel attached.
